1. general
This Privacy Statement describes how DFH Consulting, trading as "Deeply Feeling Humans" (hereinafter "the Practitioner") respects and protects the privacy of website visitors (hereinafter "the User") and describes what information is collected. This document also describes how the Practitioner collects, stores and uses data in their role as a complementary/alternative healthcare provider. Where a User makes use of the Practitioner's services, they are referred to as "the Client".
2. consent and privacy statement
When using the Practitioner's website, this Privacy Statement is brought to the attention of every User and their agreement is requested. Continued use of the website constitutes agreement with the use of cookies and this Privacy Statement.
The Practitioner requests that the User reads this Privacy Statement in full before using the website. If the User does not agree, they are requested not to use the website and to leave immediately.
3. nature of information collected
The Practitioner collects information about Users of this website for the purposes described below.
3.1 online bookings and enquiries
For bookings and enquiries via the website, personal data such as name, address, age, gender, email address and telephone number may be stored.
3.2 newsletter or information requests
For information requests or newsletter sign-ups, personal data such as name, address, age, gender, email address and telephone number may be stored.
3.3 clients of complementary care services
The Practitioner collects more data from a Client than from a standard User. For every Client, the Practitioner is legally required to maintain a comprehensive client file. See article 3.4.
3.4 client file
The client file contains the following data: name, address, date of birth, gender, email address, telephone number, medical data, notes on the (mental) health condition of the Client, and records of examinations and treatments carried out. The treatment agreement is also included in the client file. With the explicit consent of the Client, the Practitioner may request medical data from another healthcare provider or GP. Such data is also included in the file.
3.5 email messages
Email messages and related information may be stored in the context of the purpose for which they were sent.
4. purpose of storing information
4.1 administration
Information from articles 3.1 and 3.3 is stored for 7 years and used for processing bookings, delivering services, invoicing, accounting and handling disputes.
4.2 service improvement
Information from articles 3.2, 3.3 and 3.5 is used to improve the website and optimise user experience.
4.3 marketing
Information from article 3 (excluding 3.4) may be used to inform Users of relevant products and services. Sensitive data such as payment information is treated confidentially and never shared with third parties. Personal data is only used for commercial purposes where the User is a client (GDPR basis: performance of contract and/or legitimate interest) or where unambiguous prior consent has been given in accordance with the GDPR.
4.4 client file
The Practitioner is legally required to maintain a client file for every Client. Data in this file is retained for 20 years. The data may be used for:
(i) Financial administration — for invoicing purposes.
(ii) Anonymised peer review (intervisie) — the Practitioner may discuss cases with a colleague therapist on an anonymised basis, with no personally identifiable information shared.
(iii) Informing other healthcare providers — when the Practitioner is absent and a substitute is covering, or when referring to another therapist (only with the Client's consent).
(iv) Any other sharing of the client file will only take place with the Client's explicit consent.
4.5 care invoice (zorgnota)
The Client receives a care invoice after each treatment. This invoice contains: name, address, date of birth, cost of the session, description of care provided, and date of treatment. This information can be submitted to a health insurer if the treatments qualify for reimbursement.
5. changes to this privacy statement
The Practitioner may update this Privacy Statement in the future. Changes will be published on the website. Users who do not agree with changes may submit their objection via the contact details in article 9. Continued use of the website constitutes acceptance of any changes.
6. accessing, changing or deleting your data
If you wish to access, amend or delete data held by the Practitioner, or if you believe your data is being used for a purpose other than that for which it was provided, please contact the Practitioner via post or email as stated in article 9. The Practitioner will make the necessary changes or deletions, except where data is legally required to be retained.
7. security
The Practitioner uses standard technologies to ensure security and prevent unauthorised access as far as possible. However, complete security cannot be guaranteed. The Practitioner is not liable for unauthorised use of or access to data as described in article 3. Users are advised to change passwords regularly and use a reliable and secure browser.
8. keeping your contact details up to date
Users and Clients are required to inform the Practitioner of any changes to their address or other contact details. The Practitioner will assume the last known details are correct until notified otherwise. Any miscommunication arising from outdated details is the responsibility of the User or Client.

1. algemeen
Dit Privacyreglement beschrijft hoe DFH Consulting, handelend onder de naam "Deeply Feeling Humans" (hierna de 'Therapeut') de privacy regelt en eerbiedigt en welke informatie over de Gebruiker wordt verzameld. Huidig document beschrijft ook hoe de Therapeut gegevens verzamelt, opslaat en gebruikt in de functie van alternatief/complementair zorgverlener. Indien de gebruiker van de website gebruik maakt van de diensten van de Therapeut, wordt naar de gebruiker van de website gerefereerd als de 'Cliënt'.
2. toestemmingsverzoek en voorhouden privacyreglement
Bij gebruik van de website van de Therapeut wordt dit privacy statement bij iedere Gebruiker onder de aandacht gebracht en gevraagd hiermee akkoord te gaan. Verder gebruik van de website betekent dat wordt ingestemd met het gebruik van cookies en huidig privacy statement.
De Therapeut verzoekt de Gebruiker dit Privacyreglement voor gebruik van de website eerst in zijn geheel te lezen. Als de Gebruiker niet akkoord gaat, wordt verzocht de website niet te gebruiken en onmiddellijk te verlaten.
3. aard te bewaren informatie
De Therapeut verzamelt informatie over de Gebruikers van deze website voor de hieronder aangegeven doeleinden.
3.1 online boekingen en aanvragen
Voor boekingen en aanvragen via de website worden persoonsgegevens bewaard als naam, adres, leeftijd, geslacht, e-mailadres en telefoonnummer.
3.2 nieuwsbrief of informatieverzoeken
Voor het opvragen van informatie of aanmelden voor een nieuwsbrief worden persoonsgegevens bewaard als naam, adres, leeftijd, geslacht, e-mailadres en telefoonnummer.
3.3 cliënten van complementaire zorg
De Therapeut verzamelt meer gegevens van een Cliënt dan van een standaard Gebruiker. Voor elke Cliënt is de Therapeut wettelijk verplicht een uitgebreid cliëntendossier bij te houden. Zie artikel 3.4.
3.4 het cliëntendossier
In het dossier staan de volgende gegevens: naam, adres, geboortedatum, geslacht, e-mailadres, telefoonnummer, medische gegevens, aantekeningen over de (geestelijke) gezondheidstoestand, en gegevens over uitgevoerde onderzoeken en behandelingen. De behandelovereenkomst wordt tevens opgenomen in het dossier. Met nadrukkelijke toestemming van de Cliënt kan de Therapeut medische gegevens opvragen bij een collega zorgverlener of huisarts.
3.5 e-mailberichten
E-mailberichten en bijbehorende informatie kunnen worden opgeslagen in het kader van het doel waartoe deze dienen.
4. doel bewaren informatie
4.1 administratie
Informatie uit artikel 3.1 en 3.3 wordt 7 jaar bewaard en gebruikt voor het afwikkelen van boekingen, levering van zorg, facturering, boekhouding en afhandeling van geschillen.
4.2 verbetering diensten
Informatie uit artikel 3.2, 3.3 en 3.5 wordt gebruikt om de website te verbeteren en het gebruikersgemak te optimaliseren.
4.3 marketing
Informatie uit artikel 3 (met uitzondering van 3.4) kan worden gebruikt om Gebruikers te wijzen op relevante producten en diensten. Privacygevoelige gegevens worden vertrouwelijk behandeld en niet aan derden beschikbaar gesteld. Persoonsgegevens worden alleen voor commerciële doelen gebruikt als een Gebruiker Cliënt is (AVG-grondslag: uitvoering overeenkomst en/of gerechtvaardigd belang) of als daar vooraf ondubbelzinnig toestemming voor is verleend.
4.4 het cliëntendossier
De Therapeut is wettelijk verplicht van elke Cliënt een dossier bij te houden. De gegevens blijven 20 jaar bewaard. De gegevens kunnen worden gebruikt voor:
(i) Financiële administratie — voor het opstellen van een factuur.
(ii) Geanonimiseerd bespreken tijdens intervisie — zonder naar een persoon herleidbare informatie.
(iii) Informeren andere zorgverleners — bij waarneming of doorverwijzing, uitsluitend met toestemming van de Cliënt.
(iv) Indien het dossier voor een ander doel wordt gedeeld, geschiedt dit altijd met toestemming van de Cliënt.
4.5 informatie op de zorgnota
De Cliënt ontvangt een zorgnota na elke behandeling. Deze bevat: naam, adres, geboortedatum, kosten van het consult, omschrijving van de gegeven zorg en datum van de behandeling. Deze informatie kan worden ingediend bij een zorgverzekeraar indien de behandelingen in aanmerking komen voor vergoeding.
5. veranderingen aan de privacyverklaring
De Therapeut kan dit Privacyreglement in de toekomst wijzigen. Wijzigingen worden op de website gepubliceerd. Gebruikers die niet akkoord gaan kunnen bezwaar indienen via de contactgegevens in artikel 9. Verder gebruik van de website wordt beschouwd als akkoord met de wijzigingen.
6. wijzigingen gegevens of vragen over privacy
Als de Gebruiker of Cliënt de door de Therapeut bewaarde informatie wil wijzigen, inzien of laten schrappen, of indien de informatie wordt gebruikt voor een ander doel dan waarvoor deze is verstrekt, dient dit te worden gemeld via de contactgegevens in artikel 9. De Therapeut zorgt voor aanpassing of schrapping, met uitzondering van wettelijk verplicht te bewaren gegevens.
7. veiligheid
De Therapeut gebruikt standaardtechnologieën om de veiligheid te waarborgen en toegang van onbevoegden te verhinderen. Volledige zekerheid kan echter niet worden gegarandeerd. De Therapeut is niet aansprakelijk voor onbevoegd gebruik van gegevens. Gebruikers worden geadviseerd wachtwoorden regelmatig te wijzigen en een betrouwbare browser te gebruiken.
8. adres gebruiker website
Gebruikers en Cliënten zijn verplicht de Therapeut op de hoogte te stellen van adreswijzigingen of wijzigingen van andere contactgegevens. Zolang geen wijziging is doorgegeven, worden de laatste bekende gegevens als juist beschouwd. Miscommunicatie hieruit voortvloeiend is voor rekening van de Gebruiker of Cliënt.

10.2 Indien de Gebruiker of Cliënt een foto of afbeelding deelt, wordt deze tevens opgeslagen als beschreven in artikel 4.
10.3 Indien gebruik wordt gemaakt van software die informatie zelfstandig deelt zonder inmenging van de Therapeut, kan de Therapeut niet instaan voor de wijze waarop die software met de informatie omgaat.
10.4 Indien de Therapeut gebruik maakt van diensten van een derde partij die fungeert als verwerker, heeft de Therapeut een Verwerkersovereenkomst met deze partij afgesloten. Dit geldt onder andere voor Zoom (online sessies) en e-maildiensten.
